Live 3D Navigation Guides Full-Arch Dental Implants in LA

West Hollywood – October 30, 2026 -- Los Angeles patients who struggle with slipping dentures, or who have been told they lack the bone for implants, can be evaluated for fixed full-arch teeth planned around their own anatomy at Sunset Oral and Facial Surgery.

Board-certified oral and maxillofacial surgeon David Salehani, DDS, MD, pairs a cone beam CT (CBCT) scan and digital treatment planning with the X-Guide dynamic navigation system for All-on-X treatment. He maps the position, angle and depth of each implant before surgery, then tracks the drill against that plan in real time as the implants go in. The practice serves patients throughout Los Angeles, including West Hollywood and Beverly Hills.

All-on-X anchors a fixed bridge to four to six implants per arch

All-on-X is a general term for restorations that replace an entire upper or lower row of teeth with a fixed bridge. The best-known version, the All-on-4 concept, uses two upright implants in the front of the jaw and two angled toward the back. The angled rear implants engage denser bone toward the front of the jaw, so many patients with reduced bone volume can proceed without a separate bone grafting procedure.

In most cases, a temporary fixed set of teeth is attached on the day of surgery. The permanent prosthesis follows several months later, once the implants have integrated with the bone.

Navigation works like a GPS route for the drill

The angled implants are where precision matters most. The back of the upper jaw sits beneath the maxillary sinus, and the lower jaw carries the inferior alveolar nerve, which provides sensation to the lip and chin. Each implant must also sit correctly in relation to the others so the final bridge fits and functions.

During surgery, the CBCT-based plan appears alongside a live view of the drill's position. Dr. Salehani follows the planned trajectory and adjusts as he works.

Candidacy gets decided at consultation

Full-arch treatment is designed for adults who have lost most or all of their teeth in one or both arches, whose remaining teeth are failing, or who are tired of removable dentures that slip, rub or require adhesive. Patients told elsewhere that they do not have enough bone can also explore it.

At the consultation, Dr. Salehani reviews the CBCT scan, bone volume, sinus anatomy, bite and overall medical health. Some patients are better served by individual implants, bone grafting before placement or a different number of implants per arch.

A dual-degree surgeon performs the procedure under IV sedation or general anesthesia

Dr. Salehani holds both dental and medical degrees and completed a general surgery internship. He reviews each patient's medical history as part of surgical planning. His practice also covers bone grafting, sinus lift procedures, corrective jaw surgery and facial reconstruction, so he can consider the jaw as a whole in a full-arch case.

The plan starts with the CBCT scan, Dr. Salehani said, because it shows how much bone is available and where the sinus and nerve sit. "Not every patient is a candidate for All-on-X," he said. Individual results vary by patient.
